Essential Fatty Acids (EFAs) are necessary fats that humans cannot synthesize, and must be obtained through diet. Omega-3 and Omega-6 fatty acids are the two main groups of essential fatty acids. Dr. Hasler: The primary vitamins which have been linked with heart health that women may not consume enough of to protect against heart disease include vitamin E and folic acid.
WA: What does Vitamin E do to ensure heart health?
Dr. Hasler: The primary role of vitamin E in heart health is likely through its antioxidant activity. It is particularly important in protecting cell membranes from oxidative destruction by interrupting free-radical generated chain reactions in tissues and thus, preventing lipid peroxidation (and destruction of cell membranes) in those tissues. Several prospective epidemiologic studies support the hypothesis that vitamin E reduces the incidence of cardiovascular disease, including the Nurses Health Study, which showed a 34% decrease in the risk of having a cardiac event in those taking vitamin E supplements and the Iowa Women`s Health Study, which found a 47% reduction in cardiac mortality in those taking vitamin E supplements.
However, results of randomized, controlled clinical trials have not found consistent benefits. Although the Cambridge Heart Antioxidant Study (CHAOS) found a 47% reduction in fatal and nonfatal myocardial infarction in patients with proven coronary atherosclerosis in those given 400 IU or 800 IU of vitamin E daily, there was no effect on mortality. Similarly, the Heart Outcomes Prevention Evaluation Study (HOPE), which followed 9,500 people age 55 and older who were at high risk for cardiovascular events such as a heart attack for 5 years, found no difference in the rates of heart attack, stroke or death from heart disease, unstable angina, or heart failure in those given 400 IU vitamin E per day. However, these individuals had established heart disease; vitamin E may work more effectively in the prevention rather than treatment of established heart disease. Recently, the Harvard School of Public Health issued a recommendation that 400 IU of vitamin E per day should be adopted by those concerned about heart health.
WA: What does the B vitamin, Folate, do to ensure heart health?
Dr. Hasler: Folate is a water-soluble vitamin that plays a key role in a number of important metabolic processes, including the metabolism of the amino acid homocysteine. Many studies have shown that elevated homocysteine concentrations are associated with an increased morbidity and mortality due to cardiovascular disease. For example, only about 5% of the general population have high homocysteine levels (above 15 micromoles/L) whereas approximately 50% of patients with vascular disease have elevated homocysteine. Homocysteine is thought to increase cardiovascular disease risk by promoting atherosclerosis, endothelial dysfunction and oxidative stress. Folic acid supplementation has been shown to result in lower plasma total homocysteine levels and significant improvements in endothelium dependent dilation.
WA: Are there additional supplements which can help women maintain their heart health?
Dr. Hasler: Omega-3 fatty acids have been shown to reduce a number of key risk factors for heart disease, including total cholesterol and triglycerides. In addition a number of large-scale intervention studies have shown reduction in serious cardiac endpoints following consumption of omega-3 fatty acid supplements, including fatal and non-fatal myocardial infarction as well as sudden cardiac death. Based on this evidence, the FDA now allows a qualified health claim on dietary supplements of omega-3 fatty acids which states: The scientific evidence about whether omega-3 fatty acids may reduce the risk of coronary heart disease (CHD) is suggestive, but not conclusive. Studies in the general population have looked at diets containing fish and it is not known whether diets or omega-3 fatty acids in the fish may have a possible effect on a reduced risk of CHD. It is not known what effect omega-3 fatty acids may or may not have on risk of CHD in the general population. Triglyceride reduction with EPA and DHA supplements (1000 mg/day) is thought to be one of the main effects of fish oils yielding cardioprotection.
WA: What other nutritional steps can women take as preventative measures to ensure heart health?
Dr. Hasler: Women can consume adequate fiber. A recent study published in the January 2nd issue of the Journal of the American College of Cardiology found that females age 45 and older who ate the most fiber--about 26 grams daily--were less likely to develop heart disease and suffer a heart attack than women with the lowest fiber intake, about 18 grams a day over a 6 year period. The study involved more than 38,000 female health professionals. These findings support recommendations that 25 to 35 grams per day of dietary fiber may provide a primary preventive effect against heart disease. Effects of soluble and insoluble fiber were similar.
