Based on the research I've done - studying reports from various women who own the BCBGirls Kosty Pump I've not found one report of needing to return for a different size. My recommendation - based on that information - is buy true to size.

Another note - the Kosty Pump is known to be extremely comfortable, and not require a "break-in" time in order to fit correctly or to be worn for a length of time.

However, if you do end up needing to return the shoes for a different size, Zappos.com has the best return service around. So if you want to make absolutely sure you get the right fit, I always recommend them.
