The CAQH provider application is quite involved, but necessary to get credentialed or re-credentialed with insurance companies.
Today, in order to participate with most insurance companies, you must have your CAQH application completed & approved. It is an online application and the website can be a little confusing.
Although you can print the CAQH provider application out, complete it, and mail it in, I have talked to several people who never heard back after they sent it in. We find it best to attempt the online version.
Overall, the idea is a good one and saves completing many lengthy applications, but if you need help completing the form, you won't get much help by calling the Council for Affordable Quality Healthcare (CAQH).
