What makes a Baseball player a Five-tool player?
Baseball, America's pastime, is an game. Often people will say it is boring to watch. To that I cannot fully disagree; however, there are amazing subtleties about the game which require being around the game for a long while.
There are scouts, coaches, colleges, semi-pro, and professional organizations all around the world that are looking for the proverbial 'Diamond in the rough' or also known as the Five-tool player.
In fact most players don't even have 3 of these tools. If a player does possess more than 3, they usually (barring injuries) will advance to the next level.
In theory, a baseball player that can hit the ball for power, run fast around the bases, throw the ball far, hit the ball for base hits consistently, and catch the ball when it is hit to them, they will be at the absolute top of rung at every level.

What are the attributes of a baseball player Five-tool player?
A baseball player that is said to have all five tools is scouting and coaching terminology for a baseball player that has all of the attributes that make a player a 'star' or make a player a threat to the opposing team or pitcher.
These are the five tools (not in order):
1) Hit for average
2) Hit for power
3) Speed (on the bases)
4) Power
5) Fielding prowess
It is quite rare for a player to possess all of these tools, however any combination of the 5 is usually quite adequate to be a competitive player.
