Since the death of Micheal Jackson, many have talking about one of the drugs that was a common part of his life - Propofol. What is it that makes this drug such a concern,and indeed a shocking part this famous person's demise?
The first thing that shocked me, even as a health care practitioner, is the fact that the "rich and famous" are using such a powerful and potentially dangerous drug to regulate sleep.
The next thing I thought about was the dangers of this drug used when other potentially dangerous drugs are also being used, i.e., narcotics, barbiturates, and amphetamines, all of which were also part of his regimen.
Propofol or Dipivan is a powerful drug used as an anesthetic to produce deep sedation. According to the American Society of Anesthesiologist (ANA) and the American Association of Nurse Anesthesiologist (AANA), Propofol should only be administered by persons trained in the administration of anesthesia. This person must be trained to "rescue" a patient from a deeper level of sedation than expected. The practitioner must be able to control the airway under deep sedation, which means they must be able to intubate and oxygenate the person with great skill and proficiency. They must also be proficient at Advanced Life Support.
Propofol is a drug which can cause rapid, profound changes in the person it is administered to. There is no drug that can be administered to reverse the effects of Propofol. This makes it a drug which can quickly end a life if not managed correctly.
The person sedated with Propofol should be continually monitored, without interruption, to assess level of consciousness, and to identify early signs of hypotension, bradycardia, apnea,
airway obstruction and/or oxygen desaturation. Ventilation, oxygen saturation, heart rate and
blood pressure should be monitored at regular and frequent intervals. Monitoring for the
presence of exhaled carbon dioxide should be utilized when possible, since movement of the chest will not dependably identify airway obstruction or apnea.
Was this kind of care followed by the person or persons administering this drug? Was malpractice involved in this case? Probably.
Why was this drug available to Michael Jackson? What were the motives behind using this drug in this situation. Surely, this was out of the context for the drugs intended use.
