THE TRUTH ABOUT BUNIONS AND [FOOT PAIN]
[Foot Pain] from bunions is a very common complaint at my office. I once had a distasteful dream about a bunion. In this "work-mare" my foot had a bunion that kept getting bigger and bigger each time I looked at it. Perhaps I see too many bunions from day to day. However, I am surprised that many of my patients do not even know what bunions are or what causes them. Simply stated, a bunion is a bump at the base and side of the great toe

The main cause of bunions is an imbalance across the great toe when walking. X-rays with bunions almost always show the long bones behind the great and pointer toe are splayed too far apart. Over years the extra motion and stress leads to joint instability and extra bone forming and cause [foot pain].

Wearing tight shoes does not always cause bunions, but it certainly contributes to [foot pain]. Bunions are much more common in women. Our society encourages women to wear tight pointed toe dress shoes, heels, and pumps (cowboy boots receive honorable mention). However, I have seen patients as young as 10 years old with bunions. I refuse to believe that a 10 year old has been wearing the wrong shoes.

Bunions are not inherited, but do tend to run in families. What is usually inherited is a poor or faulty foot type. Bunions may be associated with flat feet, pronated feet, neuromuscular problems, or even rheumatoid arthritis leading to [foot pain].
There are many conservative treatment options for bunions that can relieve a patient's [foot pain]: padding, wider shoes, physical therapy, paring of calluses, foot orthotics (prescription arch supports), toe exercises and even steroid injections. For some older patients, I relieve pain by simply cutting a hole in their shoes. Adults should be wary of purchasing a toe splint to wear at night. I have seldom seen a splint relieve bunion/foot pain. Splints can work in adolescents, where the bone is still adapting. Splints can also be useful after surgery to maintain soft tissue correction.

Bunions are a progressive deformity. This means that over time the bump will continue to grow, unless you quit walking. The great toe will continue drifting towards and under the pointer toe until the pointer toe is cocked upwards. The good news is that a bunion may take up to sixty some years in the making. Avoiding tight shoes and wearing orthotics may slow the progression of this deformity and [foot pain].

Surgery may be considered for patients who do not respond to conservative measures. A wide range of surgical procedures is performed based on the angular relationships of bone and extent of arthritis. All procedures involve removing the boney bump from the great toe. Procedures differ on how to realign the bone behind the bunion and recovery time eliminating [foot pain].
Several studies have shown that about 85-90% of patients are satisfied with their bunion surgery and no longer have [foot pain]. On the other side of the coin, 10-15% of patients are not happy with their surgical outcome. Therefore, surgery is a last resort and should be avoided for solely cosmetic reasons.
