- Login to your PayPal account as normal so that you are on your account overview page.

- Click on 'Profile' on the bar at the top.

- In the list titled 'Selling Preferences', find 'Custom Payment Pages' and click.

- Click on the 'Options' tab to bring up the 'Customise Your Payment' page.

- Scroll down the page to the 'Add a customer service survey' option and click the tick box to the left of the text.

- Spaces will appear for you to fill in. Type in your question, and upto 5 answer options in the appropriate blank boxes. I wrote 'How did you find The Wellington Boot?' for the question, and for the answers I wrote 'Friends/family', 'Etsy search', 'Blog', 'Search Engine' and 'Twitter/Other'.

- If you see any other options on the page which you would like to incorporate on your payment page, add them now.

- Click 'Save' underneath.

- After a customer has paid, you can then find out how they answered your question by going to your account 'Overview' page (the page which appears after you log on), and finding the customer's purchase transaction in the list.
Then click 'Details' in that row.

It is not compulsory for your customer to answer your survey but I've found that most people do :-)

- Near the bottom of the 'Transaction Details' page, you will see 'Survey Details' (below 'Payment Type'). Next to this, there will be the customer's answer.
