While researching 40 different finger oximeters, 11 manufacturers, and 18 different online stores a few things became apparent. All FDA approved finger oximeters have an accuracy of plus or minus 2 for SpO2 readings between 99% - 80% and any reading under 70% is unreliable. So who makes the best oximeter? It depends on what your looking for. Here is a summary of findings with review links for further oximeter details. Out of 40 finger oximeter reviews, the median cost is $140.69. The CMS 50L was the cheapest finger oximeter and the Nonin Onyx II 9560 was the most expensive. With 93 positive internet oximeter reviews, Octive Tech 300C had significantly the most comments. The best finger oximeter by scientific validation is the Onyx II 9550. For an aviation pulse oximeter, the Nonin Nonin Onyx 9500 and Onyx II 9550 are the only available finger oximeters approved for use aboard military aircraft validating their dependability. The Nonin Sport Stat is the best unit for use in an outdoor, active, rigorous, extreme contexts and environments according to the American Alpine Institute who tested the unit in the extreme environments of the Himalayas, Alaska, and Kilimanjaro. With a basic design and an affordable price, the unit is useful for athletes to monitor their oxygen and heart rate levels during altitude sports such as climbing and mountaineering. Although it is not recommended for medical applications if you are interested in avionics or physical activities, the Octive Tech C1-B finger pulse oximeter will fit your basic needs. The oximeter can function in a variety of climates, being able to operate at temperatures between 10C to 40C (50F-104F) and humidity of 35%-80%. The accuracy of this model over all is accurate up to +/-2% for oxygen saturations from 99%-80% and heart rates between 30-255 bpm, which is the range most athletes and avionic enthusiasts would be concerning themselves with since anything below 90% depicts severe hypoxia in the average person. The oximeter will perform spot checking but is not effective in continuous monitoring and will only properly fit adults. No website offered information regarding any FDA approval for the finger pulse oximeter. The C1-B does include a few accessories such as a wrist cord and case to protect the device when nit in use. Octive tech includes a 90 days warranty with the base model and offers a one year extension with clinical guard offering an additional extension up to 2 years. Overall this unit is a decent choice for the non-medical user wishing to purchase an affordable finger pulse oximeter. Considering the free shipping offered, as of 7/6/09, the Octive tech C1-B appears to be the most affordable oximeter offered in the $40-$50 range.

The 300C is a step above Octive Tech's C1-B finger pulse oximeter. Overall this is an excellent unit and one of the most affordable finger pulse oximeters rated by the FDA as medical equipment. Reviews for this unit have been positive. Amazon product reviews rates the unit 4.5 out of 5 stars and customers from clinical guard rate this unit a 5 out of five stars. Like the C1-B, this unit can be used to spot check heart rate and oxygen levels while engaging in physical exercise. People purchasing this unit included respiratory therapists, nurses, and people with respiratory difficulty being treated under a doctor's supervision. The accuracy is +/-2% for readings between 99%-80% and is built to reduce external ambient light from interfering with its accuracy. The LED display is large and easy to read. To conserve power the finger pulse oximeter will turn off in 8 seconds when not in use. Operating on two AA batteries, the 300C is capable of working for up to 30 hours and will warn you when batteries are running low. The 300C includes a one year warranty, a carry case and a neck cord. The price of the unit ranges from $70-$64 with ClinicalGuard.com offering it at the cheapest price and also includes a free thermometer and free shipping. Overall, I feel this is one of the best basic finger pulse oximeters currently available and rate it as the author's favorite.

Keeping the clinical setting in mind, the 300CSE is manufactured utilizing a latex/rubber free process. In a clinical setting this is important since according to the U.S Center for Disease Control and Prevention (CDC) around 10% of the population is allergic to or has sensitivity to latex and rubber. The 300PN is specially designed to spot check pediatric oxygen saturation levels for ages 2-13 years. The unit is not just shrunk to fit kid sized fingers, the firmware and chipset are customized to be utilized on a child's anatomy. Pediatrics are not little adults, their needs are much different than ours which is the reasoning to have a finger pulse oximeter specifically designed to meet their needs. The sensors in the 300PN is calibrated for a child's skin which can be as thin as 0.3 inches or 8mm. To accommodate pediatric patients, the opening is tailored specifically for small fingers. The size helps to reduce false readings due to excessive movement which can result from a unit sized for adults. When used properly, finger pulse oximeters can detect episodes of oxygen desaturation before signs of hypoxia are apparent. The 300PN also displays heart rate which can also be helpful information. After reviewing multiple websites offering the CMS 50 HHC, it appears Contec has increased the warranty length to 2 years on this unit. The CMS 50 HHC is a finger oximeter built primarily for home health care. Utilizing technology patented by Contec, the CMS 50 HHC uses a dual "see through" sensor to measure SpO2 readings and pulse rate. As a more affordable oximeter, the CMS 50 HHC lacks the accuracy to consistently monitor readings while an individual is in motion due to artifact. No information was available to support any claims of the CMS 50 HHC 's ability to perform adequately at altitude. Caution should be used if you purchase the CMS 50 HHC for Aviation or Mountaineering activities. Like Contec's CMS 50L, the CMS 50 HHC should be considered for indoor, light duty use. Affordability of the CMS 50HHC can also be an appealing factor for individuals on a fixed budget.

The CMS 50L is the cheapest pulse oximeter available today. If you're looking for a basic oximeter, the CMS50L is for you. The price does reflect the quality though. Since the unit is FDA 510(k) certified, you know the design meets the basic requirements to qualify for approval; however, do not expect much more. If you are using a finger oximeter to monitor a health condition, or to monitor o2 levels, for safety, while participating in activities such as aviation or mountaineering, then you may want to look at another oximeter that has been tested and proven. Ideally this unit is probably best suited for non-critical occasional indoor use with a stable, controlled environment and average ambient lighting. Most sites state the unit goes for $249.99 and offer a pulse oximeter sale price;

With the technology to prevent indoor ambient light from affecting accuracy, the CMS 50DL deviates less than 1%. To conserve power, the CMS 50 DL will automatically shut off in 8 seconds, when not in use. Few reviews were available for the CMS 50 DL. Marketed under the name NatureSpirit® the CMS 50 DL has a few reviews at the Wal-Mart website. All the reviews are favorable for the CMS 50 DL; however, they sound more like staged reviews than natural reviews. Retail price for the CMS 50 DL is $199.99. Illedgidly Contec will be coming out with the CMS 50 A4 which appears to be an improvement to the CMS 50 A. No finger oximeter reviews were found for the CMS 50 D. The CMS 50 D Plus is a finger oximeter with the capability to store up to 24 hours of data to be downloaded into your computer via USB port. You can also synchronize readings from the CMS 50 D Plus to simultaneously display on the finger oximeter and the computer. Included with the CMS 50 D Plus is the computer software. Data obtained from the CMS 50 D Plus can be useful to spot trends for sports, sleep studies, and chronic respiratory disorders. When not in use the CMS 50 D Plus will automatically turn off to conserve power. For reviews of the CMS 50 Plus not many were found; however, there was a lengthy discussion of both the CMS 50 D Plus and the CMS 50 E for use with sleep apnea. People who liked replacing batteries seemed to prefer the CMS 50 D Plus.

The CMS 50 E is the author's choice for the finger oximeter with the most features. Storing up to 24 hours of data, the CMS 50 E can be downloaded to a computer via USB. Computer programming for the CMS 50 E is compatible with windows XP and VISTA. Everything necessary to transfer data to your computer is included with the CMS 50 E. The lithium battery saves battery costs and can be recharged by connecting the CMS 50 E to the computer or wall. Equipped with an auditory and visual alarm, the CMS 50 E will warn you if the unit is incorrectly placed on the finger, if the batteries become low or if the readings begin to fall out of the set range. The overall features of the CMS 50 E make this unit an excellent selection for sleep studies identifying conditions such as sleep apnea. CPAP talk forum has quite an extensive thread on the CMS 50 E. Comments were very positive for the CMS 50 E with one exception, it appears the CMS 50 E will not alarm outside of settings and record simultaneously. A few stores offer the CMS 50 E, with prices ranging from $199 and $135. The best deal for the CMS 50 E was at Clinical Guard. Clinical Guard customer service was found to be exceptional and knowledgeable.

The OxyCheck is manufactured by MedAir, another subsidiary of Nonin. Like all Nonin products, the Oxycheck has been extensively tested. Given the small size of all finger oximeters, dropping the unit is one of the main causes of damage. The OxyCheck is tested for durability and is able to survive drops from one meter. The Nonin Onyx pulse oximeter is the first finger pulse oximeter ever produced, receiving FDA clearance in 1995. Since being produced, the 9500 has helped to establish Nonin's dedication to a quality product. The 9500 pulse oximeter is the first in the Onyx series of three. Designed to fit many finger sizes, it can be used for both an adult and a pediatric oximeter. Although the unit itself is small, it has proven itself to be both accurate and sturdy through rigorous testing by both Nonin and third party testers. The Onyx 9500 and 9550 fingertip oximeters from Nonin Medical are the only fingertip oximeters approved for use aboard U.S. military aircraft. To achieve U.S. military airworthiness from both the U.S. Air Force and U.S. Army, the Onyx 9500 and 9550 had to maintain performance during the military's rigorous testing. Low perfusion and skin pigmentation are two issues that can prevent accurate oximeter readings. The 9500 pulse oximeter has studies verifying its ability to perform and its low perfusion claims are backed by the FDA unlike other brands claims. History of finger pulse oximetery 

The technology of Finger pulse oximeters has progressed over seventy years. The original concept was discovered in 1930's, by German scientists when they were using, spectrophotometers (instruments that measure different wavelengths and intensities of light) to measure the transmission of light through skin. Later during the 1940's, a British researcher, Millikan utilized two wavelengths of light one green and one red to create a practical ear oxygen meter which is when the term "oximeter" was created. During the 1940's and 1950's, research continued and resulted in the Waters Company developing a system that was used in experimental studies and aviation. In 1964, a surgeon in San Francisco developed an oximeter that was later marketed by Hewlett Packard in the early 1970's. Although the device began to be used in clinical settings, it was heavy (35 lbs), expensive ($10,000), and did not allow for continuous arterial oxygenation level monitoring. The improvements made in the late 1970's resulted in advances such as the use of LED's for producing the light. As time continued, respiratory therapists and anesthesiologists began to see the benefits of pulse oximetery. Companies such as Ohmeda, Nellcor, and Novametrix continued making significant advances producing multiple site probes, reducing size and cost bringing us forward to the finger pulse oximetery devices we have today.

How does a finger pulse oximeter work? 

A finger pulse oximeter uses the red color of blood to measure the percent of oxygen that is saturated within an individual's blood. Red blood cells are red because they contain protein chemical called hemoglobin. Hemoglobin contains iron, which is utilized for transporting oxygen and carbon dioxide. When blood passes through the lungs, oxygen molecules attach to the hemoglobin making them oxyhemoglobin. Hemoglobin releases oxygen while blood passes through the body's tissue becoming deoxyhemoglobin. The empty hemoglobin molecules then bond with the tissue's carbon dioxide or other waste gases, transporting it away. During this process, the color of blood changes. The finger pulse oximeter detects these changes through two light emitting diodes and two sensors on the opposite side of the finger. One diode sends out red light while the other sends out infrared light. Oxygenated blood absorbs light at 660nm (red light), whereas deoxygenated blood absorbs light preferentially at 940nm (infra-red). The relative absorption of light by oxyhemoglobin (HbO) and deoxyhemoglobin is processed by the device and an oxygen saturation level is reported. The body naturally delivers oxygenated blood in arteries and returns deoxygenated blood in veins. To ensure only the oxygenated blood levels are measured, Finger pulse oximeters are programmed to detect pulsetile blood, which only occurs in veins.

Finger Pulse Oximeter Limitations 

1.Poor positioning of the finger pulse oximeter.
2.Nail polish can give readings are capable false low readings
3.Any issues which may cause poor tissue perfusion such as hypovoliemia, hypotension, cold extremities, hypothermia, or cardiac arrest.
4.Some Cardiac valvular defects can cause venous pulsation resulting in a finger pulse oximeter reading venous blood and recording low SpO2 levels. Tricuspid regurgitation is one defect.
5.Finger pulse oximeters accuracy decreases as the value decreases. Readings below 70% are not reliable.
6.Cardiac arrhythmias that alter arterial pulsation can disrupt the finger pulse oximeter from recording an accurate reading.
7.Carbon monoxide poisoning causing carboxyhaemoglobin cannot be distinguished from oxygen resulting in finger pulse oximeters giving false readings of 100%. Finger pulse oximeters should not be utilized when carbon monoxide poisoning is suspected
8.Abnormal haemoglobins such as methaemoglobinaemia, for example following overdose of prilocaine, cause readings to tend towards 85%.
9.Age, sex, or skin color does not have a significant effect on finger pulse oximeter readings
