What is Goiter?

Goiter is a medical condition where your thyroid gland is abnormally enlarged.

Sometimes the enlarged thyroid gland (goiter) can become very large and can be easily visible to others.

The thyroid is a small butterfly shaped gland located in the neck just below the Adam's apple. This gland is responsible for producing hormones that regulate our bodily functions such as body temperature, rate of metabolism, heartbeat rate, mood and excitability and other processes necessary for life.

It is important to understand that goiter is not cancer and that most goiters are, in fact, benign.

Symptoms of Goiter includes but are not limited to:

=> swelling or disfigurement of the neck
=> feeling of tightness in the throat
=> difficulty while swallowing due to the enlarged thyroid pressing against the esophagus
=> difficulty in breathing, coughing and wheezing due to the goiter pressing the windpipe
=> distention of the vein in the neck
=> Symptoms of hyperthyroidism or hypothyroidism

What causes Goiter?

Goiter can be caused by the following:

Iodine Deficiency: The is considered as the main cause for goiter. Iodine from table salt, fish and other sources is necessary for the proper functioning of the thyroid gland to produce the necessary hormones for our body. If there is a lack of iodine intake, the thyroid gland will over stimulate to try to produce more hormones and in-turn enlarges to form goiter.

Pregnancy: During the first trimester of pregnancy, the body produces a hormone called human chorionic gonadotrophin (HCG), which can result in an enlarged thyroid gland.

Thyroid Cancer: Cancer may be detected as a lump or nodule in the thyroid gland and may result in goiter formation.

Multinodular Goiter: This is a disorder where many nodules develop inside the thyroid gland and causes an enlarged thyroid.

Inflammation: Thyroiditis (inflammation of the thyroid) can result in an enlarged thyroid or goiter.

Hashimoto's Thyroiditis and Grave's Disease can also cause goiter.

Goiter used to be common in the United States. Indeed, the upper Midwest was once known as the "goiter belt" in reference to the prevalence of goiter only a few generations ago.

Diagnosing Goiter

Goiter can be diagnosed by doing the following tests:

Hormone Test - A blood test to determine thyroid hormone levels will tell you if your thyroid is overactive (hyperthyroidism) or underactive (hypothyroidism).

Physical Examination - Your doctor may be able to determine enlargement of the thyroid gland by feeling your neck area for nodules and signs of tenderness.

Antibody Test - A blood test may confirm the presence of auto-immune antibodies, such as in Graves Disease, that are causing the goiter to develop.

Ultrasound of the thyroid - An imaging test of your thyroid gland that reveals the gland's size and possible presence of nodules.

Thyroid Scan - Imaging test that provides information on the size and function of the gland. This test involves the use of a small amount of radioactive material injected into a vein.

Treatment for goiter depends on a number of factors such as the size of the enlargement, symptoms and the underlying cause. Once these are determined, a plan for treatment can begin.

Treatments for goiter may include medication, surgery, radioactive iodine treatment and natural and alternative treatment options.

Radioactive Iodine Treatment: Radioactive iodine is taken orally and this destroys your thyroid cells resulting in smaller size of the goiter. Using this treatment may also result in the eventual condition of underactive thyroid (hypothyroidism).

Medication: Aspirin might be prescribed if the underlying cause of your goiter is inflammation. Small does of iodine (in the form of potassium iodine solution) can also be prescribed if the goiter is due to iodine deficiency. Surgery: Surgery, to remove all or part of the thyroid gland, may be necessary if the goiter is large and causing difficulty with breathing and swallowing or sometimes to remove nodules and certainly if cancer is present.
