It's Piano Concerto #2", written by Sergei Rachmaninoff.
Can anyone else confirm this? Playing the brief preview clip of the Hereafter End Credits Music on the O.S.T., it sounds like it's Rachmaninoff's Piano Concerto #2 in in C minor, Op. 18, 2nd movement with the Adagio sostenuto theme.
It probably sounds familiar, because according to Wikipedia.org is also the same theme used by Eric Carmen in All By Myself which was later covered by Celine Dion & Mariah Carey.
