What is Hodgkin's Lymphoma?

Hodgkin's lymphoma, also known as Hodgkin's disease, is a type of lymphoma first described by Thomas Hodgkin in 1832. Hodgkin's lymphoma is characterized clinically by the orderly spread of disease from one lymph node group to another and by the development of systemic symptoms with advanced disease. Pathologically, the disease is characterized by the presence of Reed-Sternberg cells. Hodgkin's lymphoma was one of the first cancers to be rendered curable by combination chemotherapy. Stages of Hodgkin's Lymphoma 

  • Stage 1 means there is Hodgkin's lymphoma in only 1 group of lymph nodes or lymphoma in one body organ only;  
  • Stage 2 means Hodgkin's lymphoma in 2 or more groups of lymph nodes or an organ and 1 or more group of lymph nodes.  In both cases, the two sites of lymphoma must be on the same side of the diaphragm;  
  • Stage 3 means that lymph nodes on both sides of the diaphragm have Hodgkin's lymphoma; and  
  • Stage 4 means that many groups of lymph nodes contain Hodgkin's lymphoma and it has spread to body organs such as the liver, bones or lungs

  • The letter "E" is used after the stage to designate disease disease spread to other areas (bone marrow, lung, etc.)

    Two other terms are used in staging:

    Progressive: The disease progresses (grows) while the patient is in treatment (usually treatment reduces tumors).

    Recurrent: Recurrent disease means that the cancer has come back after it has been treated. It may come back in the area where it first started or in another part of the body.


    Is it Hodgkin's Disease or Hodgkin's Lymphoma? 

    Let's clarify

    Hodgkin's Lymphoma was initially called Hodgkin's Disease after Thomas Hodgkin, a British pathologist who discovered Hodgkin's disease, a type of blood cancer, in 1832.

    Due to confusion of Hodgkin's Disease not being acknowledge as a type of cancer, its name was changed to Hodgkin's Lymphoma because it is a type of Lymphoma. Many in the cancer community still refer to it as Hodgkin's Disease. However, the term Hodgkin's Lymphoma is now the widely acceptable term.

    Types of Hodgkin's Lymphoma

    Nodular sclerosis (NS)
    The lymph nodes in the lower neck, chest and collarbone usually contain normal and reactive lymphocytes and Reed-Sternberg cells separated by bands of scar-like tissues. NS accounts for 60-70% of Hodgkin's cases. NS appears to account for the increase in Hodgkin's cases in recent years.

    Lymphocyte predominance (LP)
    The lymph nodes are composed largely of reactive lymphocytes and malignant L&H cells which have a "popcorn" appearance and very few Reed-Sternberg cells. LP accounts for 5% of Hodgkin's cases and affects more men than women.

    Mixed cellularity (MC)
    The lymph nodes usually contain Reed-Sternberg cells and inflammatory cells. MC accounts for 20-30% of Hodgkin's cases.

    Lymphocyte depleted (LD)
    There are two different variations of this classification: one with sheets of differing malignant cells; the other with few Reed-Sternberg cells and lymphocytes with scar-like tissue. Accounts for about 5% of Hodgkin's cases.

    Nodular lymphocyte predominant (NLP)
    Hodgkin's lymphoma - while the 4 types above are "Classical" types, NLP is in a category of its own. Typical Reed-Sternberg cells are rare to non-existent; instead variants called L & H cells (colloquially "popcorn cells") are seen.

    What is the color ribbon for Hodgkin's Lymphoma? 

    Violet or Lime Green?

    Violet was chosen to be the color to represent Hodgkin's Lymphoma by the website CureHodgkin's and the Violet Ribbon Campaign in 2001.   However, the color lime green also represents all lymphomas including both Hodgkin's and Non-Hodgkin's Lymphoma.  There have been a few debates as to which color to wear to represent Hodgkin's Lymphoma, but the consensus is that both violet or lime green can represent Hodgkin's Lymphoma.  It is a personal choice, although violet has become very popular with Hodgkin's Lymphoma survivors who prefer their own color be represented and to also respectfully honor the Violet Ribbon Campaign founded by the late Matthew Terry.

    How You Can Help Your Doctor 

    A List

    1. Pay attention to your presenting sign(s) and symptom(s). Pay attention to the location, duration, and character of the discomfort. Also record what brings the symptom(s) on, what relieves them, and what makes them worse.

    2. List past and current medical diagnosis such as other malignancies, renal failure, hypertension, heart failure, or diabetes.

    3. List past surgeries, diagnostic procedures (mammogram, chest X-rays, CT scans, etc.), and hospitalizations. Bring all diagnostic films, clinical and hospitalization records, and procedure reports if you have.

    4. List all medications and their dosing information. You should include all prescription and over-the-counter medications, dietary supplements, herbs, and vitamins. If you have any questions about the medications you are taking, bring the medications with you to show your physician.

    5. List all drug and food allergies if you have. Describe the detailed symptom(s) (skin rash, edema, etc.) of the allergy to your physician.

    What Are Hodgkin's Lymphoma Symptoms? 

    -Non painful swelling lymph nodes.
    -General fatigue and lack of energy
    -Sudden weight loss sometimes over 10%
    -Unstable fever that can be accompanied by chills for more than one week
    Night sweats
    -Itching without any rush or cause that can be seen on the skin. Some people may see some rushes or lesions on the skin but it is not common.
    -Some people may have lower back pain
    -Some people will have painful lymph node after alcohol consumption.

    Hodgkin's Lymphoma Cure Rate

    The survival rate is generally 90% or higher when the disease is detected during early stages, making it one of the more curable forms of cancer. Hodgkin's lymphoma is one of the handful of cancers that, even in its later stages, has a very high cure rate, in the 90's. Most patients who are able to be successfully treated (and thus enter remission) generally go on and live long and normal lives, due to a remission success rate of 90% to 95%.

    Strategies to make dealing with cancer easier

    A diagnosis of cancer can be extremely challenging. Remember that no matter what your concerns or prognosis, you're not alone. The following strategies and resources may make dealing with cancer easier:

    Know what to expect - Find out everything you can about your cancer: the type, the stage, your treatment options and their side effects. The more you know, the more active you can be in your own care. In addition to talking with your doctor, look for information in your local library and on the Internet. The National Cancer Institute answers questions from the public and can be reached at (800) 422-6237. Or you can contact the American Cancer Society at (800) 227-2345.

    Be proactive - Although you may feel tired and discouraged, don't let others - including your family or your doctor - make important decisions for you. It's vital that you take an active role in your treatment.

    Maintain a strong support system - Having a support system and a positive attitude can help you cope with any issues, pain and anxieties that might occur. Although friends and family can be your best allies, they sometimes may have trouble dealing with your illness. If so, the concern and understanding of a formal support group or others coping with cancer can be especially helpful. Although support groups aren't for everyone, they can be a good source for practical information. You may also find you develop deep and lasting bonds with people who are going through the same things you are.

    Set reasonable goals - Having goals helps you feel in control and can give you a sense of purpose. But don't choose goals you can't possibly reach. You may not be able work a 40-hour week, for example, but you may be able to work at least part time. In fact, many people find that continuing to work can be helpful.

    Take time for yourself - Eating well, relaxing and getting enough rest can help combat the stress and fatigue of cancer. Also, plan for the downtimes when you may need to rest more or limit what you do.

    Stay active - Receiving a diagnosis of cancer doesn't mean you have to stop doing the things you enjoy or normally do. For the most part, if you feel well enough to do something, go ahead and do it. It's important to stay involved as much as you can.
