There are also Indemnity, Scheduled and Discount Card. Most common managed dental insurance plans will be either PPO or DHMO.
Through PPO, the dental fees have been negotiated by the insurance provider with the dentist and then the dentist is added to the preferred provider list.
Normally your employer will provide the list of the participating dentist for you. You have to go to the participating dentist in order to get covered.
