There are basically five places you will have to visit in order to process your passport.

Passport photos have specific requirements for size, color, what you are wearing and so on. You could create your own passport photos at home with a camera and a printer that can print photos. But, most people don't have those.
When I got my photos nine years ago, I just stopped at the electronic section of our local store while buying groceries. They took my picture and handed them to me right then and there. - To save on gas and time, you should print your DS-11 form at home. Fill it out and then take it with you. You may need proof that you were born an American Citizen. If so, you will either go online to a web site that can provide that to you, or you will have to call one. In the case of calling, you may be able to save time by typing "oregon department of health" in your favorite internet search tool. Of course, I would type in, "Idaho department of health".
Find vital records and follow the instructions which may include calling, or just filling out forms and waiting for it to show up in the mail. - Getting the application signed by a witness that you can prove through documentation that you are who you are will require going to Acceptance Facility or a Passport Agency. If you follow those links, you can find a location close to you.
- Finally, you will have to mail the paper work and wait for it to return.

Back in my day, we had to find a place to call to find out all this information. Then we would get the form home and collect the identification information by phone and by mail until we collected all the required information. The normal process for acquiring your passport will take a minimum of six weeks. If you aren't traveling for a while, you can just start the process through the U.S. Department of State for $60.00 or less.
If you use the U.S. Postal Service, you can probably do the whole thing through them. The basic starting price is $100.00 and they won't get it any quicker than using the Department of State which also starts at $100.00.
The cost starts increasing when you are going to travel and you need your passport delivery to you guarantied within a certain amount of time. Visa Headquarters has good expedited prices compared to other services including the U.S. Postal Service and the U.S. Department of State. Expedited costs will always be in addition to the basic application processing costs.
