Enhancing Person Centered Care within Long Term Care

The philosophy is built around the individual needs and desires of our elder community, and focusing on each person's care rather than the old institutional approach.
- Texas Department of Aging & Disability
- Each state has it's own regulatory agency that monitors all elder care facilities including nuring homes, assisted living, and home health agencies.
Here is the website for Texas. Click on "Service for Texans", then click on "Older Texans", and finally click on "find and compare long term facilities." At this link you can choose "QRS Search", and find locate long term care facilities by name, address, county, city, phone, etc.
This is where one can find the last survey results on any long term facility.
However, just gaining this information from a state regulatory agency does not give a true picture, for some deficiences cited can be deceiving and do not indicate deficient care because of the way regulations are set up.
When choosing a facility use these criteria:
1. Go and take a tour. Are there any odors? Is the facility clean? Observe the interaction of staff and residents. Is there a home-like atmosphere? Are the staff caring and professional? Obeserve the activities and interaction of the residents.
2. Does the facility have a "Resident Council" and a "Family Council?" Ask for the "Family Council" president and contact that person.
3. Contact & interview the local volunteer Ombudsman.
4. Investigate the ownership structure or management structure. Are there other homes owned or operated by the same company? If so, find out the condition of those homes?
5. Contact the local hospital or hospitals, and interview the "case managers" regarding their information concerning the particular facility.
6. Know who is the Medical Director of the facility, and seek out information on that particular doctor. - Alzheimers
