Fungus - Any mold that could be inhaled or ingested can aggravate the sinuses.
Drug Side Effects - Certain medications have listed sinusitus as a possible side effect.
Foreign Objects - Young kids can try to put things in their nose not knowing that they are introducing bacteria into their sinus cavity.
Bacteria - The H. Influenzae bacteria is one of the main bacterial agents to cause sinusitis.

Runny Nose - A persistent runny nose not caused by allergies is called rhinitis. This aggravates the sinuses, thus causing sinusitis.
Narrow Sinuses - A person with unusually narrow sinuses are more prone to develop sinusitis because their narrow sinuses slow down drainage of the mucuos membranes.
A Weak Immune System - Such as those affected by HIV or AIDS. The immune system cannot do it's intended job , so sinusitis can be a side effect.

So if you're allergic to chili peppers, I'd stay away from it. As with all medications and applications thereof, it isn't for everybody. Not everyone can lean their head back and inhale a nasal spray.
If you have chronic sinusitis, you can try surgery. Surgery should only be used if all other methods are exhausted. Surgery can fix some of those people that are predisposed to sinusitis,(like those with narrow nasal cavities) but not everyone will benefit from it. So if you've tried the antibiotics and the nasal sprays and you want NOTHING to do with surgery, what are your options?
