It depends. Are you living in the country? How old are you? How much time are you putting into it every day? If you are younger, or immersed in the language or you spend a few hours every day it will happen much faster. On average 6 months of immersion or 3-4 years of non-immersion is what it takes.
2. How will I know when I am fluent?
It varies but in general, if you are comfortable speaking, you can read the language well, and you understand nearly everything and you can think and dream in the language then you are fluent.
3. Do I have to spend time in language immersion?
yes, you really do in order to experience any kind of fluency.
4. Which foreign language should I learn first?
It depends on what you plan to do with it. Spanish is always useful, while French is very useful for art and literature as well as travel, likewise Chinese is becoming more relevant.

- I agree with you there - the trick is to never give up!
I have a method of learning that is perhaps a little different but I find it effective. I find teaching others what you have learned helps. I have put together some German lens in my lens, Learn German With Me

- I went to french immersion and then studied spanish in University. Learning a second language is very challenging for adults, but is worth it. I also studied how to help adults learn to speak a second language and it came with a bit of surprise that games, pictures and props you would use for children are just as effective for adults... in fact, adults need it as well. Great lens! =)

- I speak 3 languages plus my mother tongue. I have been studying two more but dropped because of lack of goal. Must say its a hard work plus, what's the most important, one MUST have a goal. And its not getting easer with the next one.
