PE as a Condition 

With physician and researchers in agreement that PE is one of the most common male sexual dysfunctions, there are still few tools to help accurately diagnose it. One of the reasons that survey tools have not been developed is due to the fact that there are several definitions for the disorder, none of which has gained common acceptance.

It is widely accepted however that PE can be broken into two categories.

The first category is primary PE.

Primary PE applies to individuals who have had the problem since they reached sexual maturity. (Postpuberty) This is most commonly found in men between the ages of 18 and 30.

The second category of PE is known as secondary premature ejaculation. This condition is typically found in men between the ages of 45 and 65. Secondary PE occurs in men that previously experienced no signs of premature ejaculation, and without explanation began ejaculating quickly.

Who is REALLY affected by PE? 

It can be argued that the male partner is not the main sufferer in a sexual relationship if he is experiencing premature ejaculation (PE). After all, he is at least reaching orgasm even if it is too soon. It is his partner who is probably not feeling gratified if the male with PE is not able to maintain an erection for a sufficient amount of time for her to reach orgasm. This condition can certainly put a strain on a relationship, especially if the PE sufferer is not willing to seek medical help for the condition.

There are many known causes of premature ejaculation, and some of them are psychological. In addition to temporary stress or depression, there may be communication issues or unresolved conflict between the two partners. Perhaps the male experienced emotional trauma at some point in childhood such as sexual assault or repression, or discovery by a parent during masturbation as a teenager. Before starting any type of treatment plan, a discussion with a counselor, psychiatrist or psychologist may be helpful.

To achieve the best possible outcome, it is recommended that one's partner be included in the counseling sessions. If it is determined there are no major emotional issues involved, simple start-stop exercises have proven to be effective for many couples. There are exercises the male can perform by himself, but it is even more effective to practice the following technique recommended by Masters and Johnson that involves both partners:

The female should slowly begin stimulation of the male partner, and stop as soon as he senses he is close to ejaculation.

The female partner should then administer a firm compression, which is uncomfortable but not painful, just behind the glans located at the tip of the penis. She should press mainly under the penis.

After the male has a feeling of control, stimulation can begin again. This process should be repeated at least 10 or more times in one session.

After several sessions of practicing this method, the couple should sit facing each other with the female's legs crossing on top of the male's. She should stimulate him by bringing the penis close to her vaginal area and stop when he senses ejaculation is near. She should then apply the squeeze method until he regains control, and then the process can be repeated. When the couple feels ready for intercourse, the female should be in the superior position so she can withdraw immediately to apply compression if needed. This method also assists the female in reaching climax more quickly.

Most men are embarrassed to even bring up the subject of PE with their doctor. If doctors would ask if there were any sexual concerns as part of a routine examination, it would go a long way in the quest to identify and treat the condition. But if a PE sufferer really cares about the happiness and satisfaction of his partner, he will find the courage to seek help. If the start-stop exercises do not work over the long-term, there are alternative therapies and medications such as serotonin reuptake inhibitors (SSRIs) that are successfully being used to treat premature ejaculation.

Ancient Chinese self massage techniques for curing PE & YOGA 

There has been a lot of press recently surrounding Ancient Chinese self massage techniques for premature ejaculation. Many different online companies are selling manuals and other information on how to do this massage. There has been no recent published clinical evidence that this massage can effectively cure premature ejaculation. On the flip side, there has been no recent evidence that it's not effective.

Alternative, natural treatments for PE can be a really great solution if the patient and his doctor find an effective one. There are some natural treatments that have been mentioned a number of times in very reputable medical journals. Yoga has proven effective in studies and also the natural topical cream ss cream has shown significant positive results during testing.

STUPID IDEAS:

Lotions and potions that 'Desensitize' your penis. C'mon, this is the most sensitive organ on your body. Do you really want to be putting things on it that make it feel less?

Silly mental games like: 'Think of baseball'-- If they didn't work in the past why would they start to work now?

Difficult 'techniques' that include 'exercises' to control orgasm.

Embarrassing 'Squeeze' techniques that have your partner squeezing your penis.

A 'start and stop' program. I am sure you have heard of this one! You know, stop every time you get close to orgasm. If you could stop it from happening then you wouldn't be reading this letter. Besides, how much fun do think it is for her?

A penis enlargement product. This will NOT enlarge the size of your penis. The companies out there claiming that they have pills that increase penis size are totally bogus! C'mon... we know it deep down, but it's that glimmer of hope that so many men out there are buying. Penis enlargement pills do NOT work...period!

Ejaculatory Reflex - What Is It? 

Ejaculatory reflex begins in the urethra of the penis and consists of many involuntary contractions which push the sperm out of the seminal vesicle and out of the penis. This reflex is controlled by the PC muscle. There are many different companies which are trying to promote exercises which strengthen this muscle which intern is supposed to cure patient's premature ejaculation problems. There has been no recent published clinical evidence that these exercises can effectively cure premature ejaculation. There are other options for patients which have been proven in clinical trials to effectively increase control over ejaculatory reflex. SSRI medications like Dapoxetine ( Prilogy%u2122 ), is one of them.

A consultation with a doctor is a recommended to find out the best way to cure a specific premature ejaculation case.

Is it Just In Your Head? 

Behavioral interventions have dominated treatment of PE, primarily because most physicians view the condition as having a psychological etiology. Pryor, however, does not believe that the only cause of PE is psychological. He stated, "%u2026for some people it may be a psychological issue and for many others clearly there's a lot of evidence that it's biological. We don't have any great diagnostic criteria to say that this is what's causing it with you vs. this is what's causing it with another patient." More study is needed in order to fully understand PE's etiology. Behavioral interventions do not work with all individuals, and those who rely on it for treatment find that it is time intensive and expensive. It is also difficult to sustain any gains over long periods of time. Pryor also does not believe that for long-term change in PE patients that behavioral interventions are the treatment of choice.

A Holistic Approach 

Acupuncture can be a powerful tool to combat PE. It has been used in the past to successfully treat a variety of sexual dysfunctions.

Traditional Chinese sexology embraces a holistic approach to sexuality, starting with the Kama Sutra. There are also references in documents dating back all the way to the Han Dynasty (206 BD) and the end of the Tang Dynasty (AD 907). The literature focuses on the equality and harmony of the sexual relationship between men and women; the act of intercourse involves the equal exchange of Yin and Yang. It is interesting to note that the literature recommends moderation, as excessive intercourse can deplete individuals of their resources.

The texts emphasize the methods in which each partner can satisfy each other in an emotionally intensive and intimate way. Desire is an indicator of overall strong health and a lack of libido can be attributed to the accumulation of negative emotions.

For patients who do not respond well to traditional therapies or who want to choose a less traditional approach, Chinese medicine could provide appropriate treatment options . Once referred, the practitioner then completes a comprehensive evaluation of the patient, including assessment of their spirit and where the optimal points for acupuncture stimulation are on their body.

The vast majority of patients can expect to be treated with basic electro acupuncture 14; electro acupuncture is the most powerful tool, producing a stronger stimulation that just a needle. The goal is to minimize pain and balance the patient's Qi. Patients can expect to feel relaxed during treatments that last between 30 and 60 minutes.

Most patients report an improvement in their PE after the initial 3-4 treatments. If the acupuncture is effective and patients wish to continue, the general course of treatment consists of ongoing sessions tapering off to an ongoing maintenance schedule of every 6 months. Chinese therapies that may complement acupuncture include herbal medicine, Qi Gong, massage and cupping.

Another approach that the acupuncturist can recommend is the Aung Conjoint Sexual Alignment Therapy. Treatment for sexual dysfunction, including PE, is complicated as it encompasses the physical, mental and spiritual aspects of an individual. Because of this, it is often thought that inviting the partner to participate in treatment can positively affect the treatment outcome. In this approach the couple lies next to each other with palms touching to allow the flow of electrical current to pass between them. They are wired and interconnected and using a low-intensity electro acupuncture protocol, both electricity and their concern for each other are allowed to flow between them and thus balance their energy for sexual satisfaction.

The use of acupuncture may be an effective holistic approach to the treatment of PE. It can help balance the energy of the individual and provide rapid results for some patients.

SS Cream - an effective treatment for PE? 

SS cream was found to be an effective treatment for men suffering from Premature Ejaculation

As Premature Ejaculation (also called Rapid Ejaculation) is a very common sexual dysfunction, researchers around the world are trying to identify potential treatments for it. In a study, conducted by Choi HK et al in Korea and published in Urology, it was found that SS cream is an effective treatment for men suffering from PE.

SS-cream is a topical agent derived from the extracts of nine natural products (Ginseng Redix Alba, Angelicae Gigantic Radix, Cistanchis Herba, Zanthoxylli Fructs, Torlidis Semen, Asiasari Radix, Caryophylli Flos, Cinnamoni Cortex, and Bufonis Veneum) for the treatment of PE. In patients with Premature Ejaculation, SS-cream is thought to decrease the penile sensitivity to vibratory stimulation and therefore prolongs latency time.

The study was conducted in three medical centers in Korea. One hundred six patients (mean age 38.7 years) completed this study. The ejaculatory latency time was measured by stopwatch and sexual satisfaction ratio of both partner and patient were investigated for a total of six treatments. Patients were instructed to apply the cream on their penis 1 hour before sexual intercourse in a double-blind randomized placebo controlled fashion (each patient received either real treatment or a dummy cream and neither patients nor doctors new which patient is receiving which treatment). The effect of the SS cream was measured by the prolongation of ejaculatory latency time and improvement of the sexual satisfaction ratio before and after each treatment.

In the screening period before patients were enrolled into the study, the mean ejaculatory latency time was about 1.4 minutes, and neither the patients nor their partners were satisfied with their sexual lives. After treatment, the mean ejaculatory latency time was prolonged to about 2.5 minutes in the placebo group and to about 11 minutes in the SS-cream group. About 80% of the patients treated with the SS cream improved by more than 2 minutes as compared with only 15% on placebo. About 82% had an improvement in sexual satisfaction compared with about 20% on placebo. In this trial, SS cream was tested 530 times (and average of over 5 times per patient). 98 cases (18.49%) resulted in a sense of mild local burning and mild pain. No adverse effect on sexual function or partner and no systemic side effects were observed.

The authors concluded that according to these results, SS-cream is effective and safe in the treatment of premature ejaculation , with mild local side effects.

Why Do I Suffer From Premature Ejaculation? 

There are TWO Fundamental Problems That Cause Men To Suffer from P.E.

First

You could be the most caring and sensitive man in the world, but women are anatomically different than men. Quite frankly it can be difficult to have a woman reach orgasm even if you DON'T ejaculate quickly.

When a man is sexually aroused, whether by sight or physical contact, hormones are released in the brain that dialate blood vessels in the penis causing an erection.

Men who ejaculate prematurely suffer from a hormonal imbalance. Studies suggest that men with low levels of a neurotransmitter called serotonin are more likely to suffer from P.E.

The cause of some men producing serotonin more slowly than others is directly related to the 'thoughts' being produced in your mind. These 'thoughts' actually reduce serotonin resulting in rapid ejaculation.

Here's the problem:

Many of these thoughts can NOT be controlled by you. It's just impossible. They may even be subconscious thoughts that you are NOT EVEN AWARE that you're experiencing.

Ejaculation is controlled by the sympathetic nervous system as opposed to the parasympathetic nervous system.

What does this mean?

It means that you have NO control over ejaculation, it's a simple reflex.

Remember when you were a kid and the doctor used to tap you on the knee with his instrument, and your leg would kick? The doctor was testing your "reflexes."

This is an example of the sympathetic nervous system. You can't control it. The moment that instrument hit your knee, chemicals were released in the spinal cord that instantaneously caused you to kick your leg up. No matter how hard you tried not to move your leg, you couldn't "will" it into not moving.

The same holds true for ejaculation. It's a reflex. Once those chemicals have been released in your spinal cord, your body is told to ejaculate. No matter how hard you try to control it, you simply can't. It's just the way a reflex is.

According to Dr. William Ganong, M.D., a noted physician, physiologist, and medical educator whose textbook entitled "Review of Medical Physiology" has taught reproductive physiology to tens of thousands of physicians over several decades, "Ejaculation is a 2 part spinal reflex that involves emission, the movement of the semen into the urethra; and the ejaculation proper, the propulsion of the semen out of the urethra at the time of orgasm."

Again, this simply means that ejaculation is a spinal reflex AND can NOT be controlled, the same way as you can't control your knee jerking when the doctor taps it! When your body is ready to ejaculate, you can NOT stop it or "will" it to stop. It's a reflex.

So for you to think you can help yourself and "think" your way into curing your P.E. is not realistic.

This is where medicine has a place.

Brain chemicals can have an inhibitory effect on spinal messages. The important chemical that can inhibit spinal cord messages is called serotonin.

For years physicians have been using medication known as SSRI's. This stands for selective serotonin reuptake inhibitors. These medications have been used most widely to treat depression and anxiety.

Interestingly, physicians have found that SSRI's effectively treat a very high percentage of men with P.E.

SSRI's increase the level of serotonin. This increase is crucial if one wants to help P.E.

In the brain, information is passed between nerve cells via a small gap between the cells called a synapse. The neuron, or nerve cell, that sends the information releases neurotransmitters, serotonin being one of them, into the synaptic gap.

The serotonin is then recognized by the recipient nerve cell. When the recipient nerve cell recognizes the serotonin, it is stimulated and relays the signal. Almost 90% of the serotonin is taken up again by the cells, hence reuptake. SSRI's delay the reuptake of the serotonin.

By delaying the reuptake of serotonin, you are effectively increasing the quantity of sertonin that the receptor cells are exposed to. This increase in serotonin can effectively treat P.E. because a message is sent from the brain to the spinal cord to "inhibit" the reflex of ejaculation.

Here's the bad news about SSRI's...

1. They are a prescription drug. Therefore, a doctor's prescription is necessary for proper dispensing of the medicine.

2. Prescription SSRI's are costly.

3. You will need to schedule an appointment with your doctor, thereby having the nuisance of a costly and time consuming office visit.

4. In order to be prescribed an SSRI, you will have to discuss the embarassing topic of P.E. with your phsyician. After sharing this very private and intimate detail about yourself, your phsyician may not even prescribe an SSRI.

FACT: If you can help stabilize serotonin levels in the brain, then you can delay your orgasm. Thereby lasting longer, creating more pleasure for your partner, and satisfying her needs.

The Problem With Many Doctors

Another problem is that, unfortunately, many physicians are prescribing the wrong medication to treat premature ejaculation which adds to the problem of visiting a doctor's office.

There is a medical difference between something known as erectile dysfunction, also known as E.D., and premature ejaculation.

Most men have seen advertisements for E.D. medication. E.D.medication is for men that have a problem getting an erection. This is not the same as premature ejaculation.

E.D. medication is not going to delay ejaculation. E.D. medication is going to increase blood flow to the penis aiding your erection.

We have made great advances in both the treatment and diagnosis of E.D. in the last ten years. E.D. was a taboo topic and often never discussed with physicians in the past.

In many ways, the topic of P.E. is similiar to where E.D. was ten years ago. Many physicians don't ask their patients about it, and many patients are too embarrassed to bring it up with their doctor.

How Did I Develop This Imbalance?
