Classification of Hair Loss 

There are many approaches to the classification of hair loss alopecia, most common being,

Cicatricial Alopecia or scarring hair loss:
Not much hope for the patient unless treatment is instituted early so as to stop the scarring which will invariably close down the hair follicles and burn down the germinating ground, leaving no scope for hair re-growth. The only alternative here is whole skin transplantation.

Non cicatricial alopecia or non scarring hair loss:
This is the commonest variety of hair loss and may be due to many causes. Such hair loss can either be diffuse or localized.

Scarring Hair Loss 

The cicatricial alopecias(scarring hair loss)are differentiated from the non cicatricial alopecias (non-scarring hair loss) by the absence of hair follicle opening, increased wrinkling of the involved skin and thin, shiny, dry surface covered with telangiectases or dilated blood vessels. Towards the periphery, hairs will be twisted due to the pull of the fibrotic tissue in cicatricial hair loss.Scarring hair loss may be caused by the following:
  • Developmental or hereditary disorders: Defective development of hair follicles and skin, as in aplasia cutis, darier's disease etc
  • Traumatic: Chemical and mechanical, as in burns or deep wounds.
  • Neoplastic: Hair loss in basal cell carcinoma and similar cancers that damage the dermal layer of the skin.
  • Follicular inflammation: Inflammation of hair follicle is seen in diseases like Discoid lupus erythematosus, tinea capitis, which heal with scarring.
  • Dermal Inflammation leading to secondary damage to hair follicle, as in morphea, skin Tuberculosis etc.

The Non-Scarring Hair Loss 

  1. Diffuse Hair Loss:
    • Commonest among diffuse or generalized is the male pattern androgenic alopecia, otherwise known as common baldness. This, actually is not a disease, but a genetically determined physiological process. This is the source of a multi billion dollar business bonanza to the drug, cosmetic, plastic surgery and wig & weaving industry!
    • Other causes of generalized hair loss include anemia due to iron or vitamin deficiency, other nutritional deficiencies, hormonal imbalances, diseases affecting endocrine glands, medications, especially those taken for the treatment of cancers, following delivery or prolonged illness, severe stress, use of harshshampoos etc.

Treatment of Common Baldness 

Medical Treatment of Common Baldness:

Topical Minoxidil is THE treatment for baldness. Most so called herbal and other miraculous cures are ineffective, or a concoction containing minoxidil itself. Fenestride is another drug effective in common baldness.

Summary of Minoxidil therapy:
  • Life long treatment
  • Though the drug is effective, it may show only a limited improvement in most people.
  • It arrests progress of hair loss quite effectively
  • Minoxidil therapy is quite expensive
  • Response to treatment is seen only after 3-4 months
  • Hair fall restarts after stopping medication!
  • Should be used carefully in those with fluctuant blood pressure and heart diseases.
  • Irritation and allergic contact dermatitis may occur to the application.
  • Application 1 ml twice daily after wash with minimal rubbing

 

Surgical Treatment of Common Baldness

Before contemplating surgery like hair transplantation, the subsequent risks and cost of such surgeries should be considered.

In milder cases, a change of hair style or later, a wig would suffice. Flap and strip grafts, scalp reduction operations are also available as alternatives to hair transplantation .

Hair weaving is a new technique that is gaining popularity.

Male Pattern Baldness in Females .

The main causes of male type baldness in females is due to excessive production of circulating androgens, resulting from diseases of various endocrine glands, like polycystic ovarian disease, adrenal hyperplasia, cushing's syndrome and adrenal and ovarian tumors.

  • Sudden , generalized hair fall should call for urgent consultation.
  • If associated with other systemic symptoms like generalized weakness, pallor, in females, menstrual irregularities etc.
