Tools of the Trade

 

Tubal ligation reversal has been a wide growing procedure in recent years, and much can be contributed to the wealth of information available on the internet. Although it is great that the word of this successful procedure has been getting out, there is also the problem of misconception of information. This page is dedicated to addressing some of the misunderstood aspects and preferences of the TR procedure, particularly the use of the stent, tubal probe, loupes, and patient care during surgery. As surgical technicians with Dr. Berger at CHSC, we have found that there are many different avenues of which a surgeon can approach a successful TR. Dr. Gary S. Berger has conducted extensive data collection and studies which has proven that his approach to this procedure works. The following are the approaches and preferences of the most experienced TR surgeon in the world.

Terms of Tubal Reversal 

Anastomosis: This is a surgical performed connection of two structures.
Lumen: Medically speaking the lumen is the inner membrane of a vessel in the body.
Proximal End: The end of the tube at the Uterus
Distal End: The end of the tube away from the uterus and closes to the Ovaries

The Stent 

The stent is an incredibly useful tool during the Tubal reversal procedure. It's a nonabsoarbable suture, with a blue pigment to enhance visibility. The moistened stent ends are placed through the proximal and distal portions of the tube whereupon completion of the anastamosis, it is removed. It has a variety of beneficial aspects. First, as an item used to secure the patenecy of the tubal lumen while the anastamosis is being sewn, and secondly acts as insurance to prevent back walling of the tubal lumen. The back walling of the tubal lumen could lead to a surgical-made occlusion of the tube. All the more reason the stent is used to assures that the tube is open for passage of the fertilized egg.

Tubal Probe 

The tubal probe is a stainless steel instrument that acts as a suture passer. It is introduced through the fimbrial end of the tube, and helps to pass the stent through the distal tube's lumen. The tubal probe does not remain in the tube, and it is removed from the stent, taking unnecessary stress off of the tube while it is being sewn.

The Surgical Loupes 

Surgical loupes have become a standard for high-resolution optics during many surgical procedures. Some to speak of are; dental (Oral), plastic, cardiovascular (Heart), and gynecological (Tubal Reversal) procedures. The loupes allows for veiwing very detailed images with a precise depth-of-field. Microscope grade lens are mounted on lightweight frames, allowing for magnification during a surgical procedure. The benefits are outstanding to include visual accuracy, precision, exceptional clarity, improved posture,and reduced eyestrain. Loupes give a surgeon a flexible system allowing him or her to adjust these critical settings precisely by a change in head position. The view is increased, sharper, crisper with a wide fields of view, and the image becomes three-dimensional; making lightweight, precision optics the preferred way in most microsurgical procedures used worldwide. Optical loupes are fast becoming an essential tool to keep the magnified area clear and accurate, this allow for quality work when the anastamosisis is being created.

Patient Care During Surgery 

Care of the skin and tissues during tubal reversal surgery aids in the healing process and promotes a speedy recovery. Dr. Berger's use of moist laps upon initial skin incision begins the care of skin and subcutaneous tissue. As he approaches deeper tissue, his use of finger retraction or hand held retractors is gentle to surrounding tissue. This use of retraction also promotes visualization for repair of the fallopian tubes. Dr. Berger reconnects the fallopian tubes in one layer, reducing scar tissue, and allowing the fertilized egg the chance to move through the tube to the uterus.
