So Easy to Use.
Once you receive your discount card in the mail, use it at any participating provider and receive the discounted price - it's that simple! You pay the discounted fee directly to the provider at the time of service. There are:
No claim forms
No deductibles or copays
No participation restrictions, regardless of your age or medical condition
And no frequency limitations - use as often as you like!

There are three ways to sign up:

Online. Complete the online form and supply your payment information for the fastest way to join!

Telephone. Call one of our customer service reps toll free at 1-877-MY-VITAL (1-877-698-4825).

Pamphlet/Brochure. You can download a brochure and enrollment form from the web, or if you received a brochure in the mail a paper sign up form is included as part of the brochure. Simply tear the form off, complete the information, include your payment and mail it to the address listed on the brochure.

Is there a cancellation policy?
Your satisfaction is important to us. If you are not satisfied with the Vital Savings by Aetna® program for any reason within the first 30 days, you may cancel your participation and receive a full refund (minus the $15 processing fee).

Please note that this is not a health insurance policy and we do not make payments directly to the providers of health services. You are obligated to pay for all health services. You will receive discounts for health services at certain health care providers who have contracted with the program. This program is administered by Aetna Life Insurance Company, 151 Farmington Avenue, Hartford, CT.
